Diagnostic Assessment
West Michigan Addiction Consultants, PC, offers a three-day outpatient assessment for the purpose of diagnosis of the conditions of addiction (chemical dependence), chemical abuse, mental illness and/or personality disorder. This assessment utilizes the expertise of health professionals who specialize in the diagnosis and treatment of addiction and mental illness. Because this assessment spans three days, and is done on an outpatient basis, the person undergoing the assessment may stay at a nearby hotel during the time of the assessment. We request that the assessment be done on Monday, Tuesday, and Wednesday with the final diagnosis and recommendation to be given on Thursday.
The following services are provided during the assessment:
- Complete Medical History and Physical Examination
- Addiction Medicine Assessment
- Biopsychosocial Assessment
- Psychological Testing and Evaluation
- Laboratory Evaluation
- Preparation and Filing of Reports
A psychiatric assessment may be added if indicated.
All reports which will be sent to others will be done so only with the written authorization of the person who is undergoing the assessment.
